As aging societies face the dual challenge of dementia and physical frailty, developing feasible and effective intervention protocols in community-based settings is crucial. This study investigates the efficacy and feasibility of a 12-week intervention combining resistance training with specialized "Foot-Core" exercises (Short Foot Exercise and toe-grip training) for frail older adults with dementia (n=13) in a day care center.The program consisted of 24 sessions (60 min/session, twice weekly). Quantitative assessments included the Short Form Berg Balance Scale (SFBBS), Hand-Held Dynamometer (HHD) for multi-muscle group strength (including hallux and lesser toes), and the Functional Independence Measure (FIM).The results demonstrated exceptional feasibility, with a mean attendance rate of 90.83% (± 13.14%), suggesting high acceptance among participants with cognitive impairment. Statistical analysis revealed significant improvements in balance (p < 0.01), strength across all measured muscle groups (p < 0.01), and functional independence (p = 0.032). These findings suggest that targeting intrinsic foot muscles alongside traditional resistance training can significantly improve physical resilience and quality of life for frail seniors. This study provides a scalable model for day care facilities to implement specialized exercise programs that address the complex needs of the dementia population.
